Urine toxic and essential elements analysis is an invaluable tool for the assessment of retention of toxic metals in the body and the status of essential nutrient elements. Toxic metals do not have any useful physiological function, adversely affect virtually every organ system and disrupt the homeostasis of nutrient elements.

Analysis of the levels of toxic metals in urine after the administration of a metal detoxification agent is an objective way to evaluate the accumulation of toxic metals. Acute metal poisoning is rare. More common, however, is a chronic, low-level exposure to toxic metals that can result in significant retention in the body that can be associated with a vast array of adverse health effects and not chronic disease. Once cannot draw valid conclusions about adverse health effects of metals without assessing net retention. For an individual, toxicity occurs when net retention exceeds physiological tolerance.   Net retention is determined by the difference between the rates of assimilation and excretion of metals. To evaluate net retention, one compares the levels of metals in urine before and after the administration of a pharmaceutical metal detoxification agent such as EDTA, DMSA or DMPS. Different compounds have different affinities for specific metals, but all function by sequestering “hidden” metals from deep tissue stores and mobilizing the metals to the kidneys for excretion in the urine. Guidelines for collection periods after administration of the most commonly utilized agents are provided in the table below:

Common Agents

DMPS (IV)

It is important to perform both pre-and post-provocation urinalysis to permit distinction between ongoing exposures to metals (pre-) and net bodily retention. The pre-provocation urine collection can also be utilized to assess the rate of creatinine clearance if a serum specimen is also submitted.

Many clinicians also request the analysis of essential elements in urine specimens to evaluate nutritional status and the efficacy of mineral supplementation during metal detoxification therapy. Metal detoxification agents can significantly increase the excretion of specific nutrient elements such as zinc, copper, manganese, and molybdenum.

Chromium metabolism authorities suggest that 24-hour chromium excretion likely provides the best assessment of chromium status. Early indication of renal dysfunction can be gleaned from urinary wasting of essential elements such as magnesium, calcium, potassium and sodium in an unprovoked specimen.

Variability in urine volume can drastically affect the concentration of elements.   To compensate for urine dilution variation, elements are expressed per unit creatinine for timed collections. For 24-hour collections, elements are reported as both unites per 24 hours and units per creatinine.

Mercury Safe Dentistry

mercury safe denstisry

The AmericanDentalAssociation actually supports Mercury-Safe Dentistry, it just uses a different name. The ADA calls it The ADA’s ‘Dental Mercury Hygiene Recommendations’. The ADA lists a number of sources of mercury exposure at the dental office, including polishing and removing amalgam. It also recommends that all personnel involved in the handling of mercury and dental amalgam be trained in the potential hazards of mercury vapor and the necessity of observing good mercury hygiene. Although limited in scope the ADA does promote mercury safe dentistry. So while it still may vehemently defend the use of amalgam it encourages dentists to be mercury-safe. Not only does the ADA encourage dentists to be mercury-safe, you should promote the fact that you are to your patients and the public. It also shows that the ADA does not believe that the elemental mercury used in amalgams is somehow safer than elemental mercury used in other products and equipment.

The ADA’s Dental Mercury Hygiene Recommendations were published in: J AM Dent Assoc, Vol 134, No.11, 1498-1499; © 2003, American Dental Association. You can access the article online at http://jada.ada.org/cgi/content/full/134/11/1498.

AESTHETIC FILLINGS

The Makeup of a Tooth
The external part of the tooth is called the enamel, which is arguably the hardest natural material produced by animals. It consists of very closely knit calcium crystals and contains no living tissue. Dental porcelains easily and efficiently imitate the enamel part of a tooth.
The inner part of a tooth is the dentin, which is more porous and similar to bone. It consists of microscopic tubes of collagen with calcium crystal deposits. Dentin is a live tissue and a very sensitive one. Feeling in the dentin is transmitted through the pulp, or nerve tissues, in the very center of the tooth. To mimic the natural dentin in the teeth, today’s dental composite resins are common in non-amalgam fillings. These composites are made of a plastic resin and silica filler.
